Fire Incident & Site Records

Use this for standalone fire incident reports, fire inspection records, or other site record requests that do not contain Protected Health Information (PHI).

Medical & Patient Records

Use this for records (ambulance/paramedic) involving Protected Health Information (PHI). If applicable, you may also request that the associated fire incident report be included in this request.
