Verification of Employment
It is important to review this section in its entirety prior to submitting your request. Requests can be made by email to firstname.lastname@example.org (preferred) or by fax to 714-738-3113. Please be advised that phone verifications can be used to confirm employment or information you already have, we do not provide any employee information over the phone.
We do not respond to requests from Equifax. Lenders should contact us directly and do so following the requirements listed on this page.
You must include the employees signed* authorization.
The employee authorization must be dated within 30 days of the request date, unless otherwise stated within the authorization.
The employee authorization must be specific to the company making the request for release of information. This means that If you are a third party requestor, the name on the release must be your company, not the company for whom you are making the request.
The employee authorization must indicate that a copy may be accepted as an original.
An email address is required for response, regardless of how the request was submitted. If an email address is not available, response will be provided via the U.S. Postal Service to the address listed on the request.
Include your request form for completion.
Include a contact name and phone number.
Include the last four (4) digits of the employee's social security number. Please do not include more than the last four (4) digits, if there is a question regarding identification we will call.
Submissions that require clicking a link within an email to complete the verification, will not be answered. For security purposes, we will not click on links from an email outside of our organization.
Allow up to five (5) business days for processing. Due to current circumstances, responses may take up to two weeks.
*Digital/electronic signatures are not accepted to authorize release of an employee's information.

Claim for Damages
To file a claim for damages fill out a Claim for Damages form and file the completed form with the City Clerk's Office.
